Service Detail

PCMHT is a collaborative and integrated multi-disciplinary service providing care and support to service users who require a level of care above that available through general medical services (GP). The service operates a single point of access for mental health care, provides time-limited interventions for patients referred from primary care as well as a step-down pathway for service users transitioning from secondary care services. PCMHT works in collaboration with, and provides consultation, advice and guidance to other healthcare and social care providers.
