  1. Primary Care Mental Health Support Service (Croydon)

    The Primary Care Mental Health Support Service (PCMHSS) works with Croydon adults residents (18-65 years) that have experienced severe and enduring mental illness and have achieved some stability in their recovery that they are ready for transfer to Primary Care. The client group will be motivated to increase self management and independence in preparing for transfer to primary care. The PCMHSS works in collaboration with the patients, carers/family and their GP practices towards full transfer from secondary mental health service to Primary Care. Our team brings together mental health practitioners including a psychiatrist that will support the transfer process.

    Patients accepted into the PCMHSS will meet certain criteria e.g. their mental health (mainly psychosis) and social situation is stable, and the prescribed medication can be managed by the doctor's surgery.

    Patients will be transferred from the service, to primary care, following a formal multi-disciplinary review with the prescribing GP, practice staff and our service.

    In the event of needing input from secondary service, the referral process will be the same as it is now for all.

  2. Primary Care Mental Health Team

    PCMHT is a collaborative and integrated multi-disciplinary service providing care and support to service users who require a level of care above that available through general medical services (GP). The service operates a single point of access for mental health care, provides time-limited interventions for patients referred from primary care as well as a step-down pathway for service users transitioning from secondary care services. PCMHT works in collaboration with, and provides consultation, advice and guidance to other healthcare and social care providers.

  3. Promoting Recovery Service (Croydon East)

    We provide care and support for people, aged 18-65, who have severe mental illness. We care and support people who live in the London Borough of Croydon.

    People eligible for our service must be on the Care Programme Approach (CPA). The CPA is a way of describing what you need in order to get the best care. It is for patients who need to see several people or organisations for their mental health care and treatment.

    We provide people with a care co-ordinator to plan their care and make sure they get the best available help. They are usually a community psychiatric nurse, social worker or occupational therapist. We also run an outpatient clinic for people who are able to live more independently.

    Our service brings together teams of health and social care professionals, including nurses, social workers and psychiatrists.

  6. Psychological Interventions Clinic for Outpatients with Psychosis (PICuP)

    Our service offers cognitive behavioural therapy (CBT) and family interventions for people with distressing positive symptoms of psychosis (including those with a diagnosis of bipolar disorder). This includes delusions and hallucinations, or those with a history of psychosis whose main difficulties are secondary emotional problems. We do not offer treatment for those who have predominantly negative symptoms, learning disabilities, are not distressed or do not want help.

    Our service is psychology-led, headed by international, clinical and academic experts. PICuP was one of the two national demonstration sites for the Improving Access to Psychological Therapies for Severe Mental Illness (IAPT for SMI) project which aimed to increase access to psychological therapies for psychosis, bipolar disorder and personality disorders.

    • include families, carers and other mental health professionals involved in the person’s care, if the patient wishes to

    • work towards improved quality of life for people with psychosis through clinical research and audits

  7. Psychology and Psychotherapy for Older Adults (Lewisham, Southwark, Lambeth and Croydon)

    The team offers a range of specialist, evidence based, psychological interventions to people aged 65 and over who are experiencing moderate or severe mental health difficulties or dementia. We also see younger people who are experiencing memory or other cognitive problems. We work with families and others caring for people with behavioural and psychological symptoms of dementia. Services offered include:

    - Cognitive behaviour therapy
    - Family therapy
    - Psychodynamic psychotherapy
    - Assessments of cognitive function and advice on strategies to manage these difficulties
    - Assessment and advice on managing challenging behaviours in the context of dementia
    - We see people in local clinics but can offer home visits

  8. Psychology in Hostels Project

    We provide a flexible and responsive psychology service to adults (18 years+) with complex needs in the Lambeth homeless hostel network. Such people may have severe mental health problems, histories of complex trauma and comorbid drug and alcohol use.

    Our service is provided by psychologists based across three Thames Reach hostels which provide a ÔPsychologically Informed Environment' (PIE) for residents and staff.

    The PIE aims to ensure that everything that takes place within the hostel holds individuals' psychological and emotional needs in mind. It provides individual and group psychological therapy; and staff training, supervision, consultation and reflective practice. By increasing clients' psychological stability this service also aims to help them sustain future independent living, engage with mainstream mental and physical health services and find meaningful occupation.

    This service is provided in partnership with the London Borough of Lambeth and Thames Reach hostels.

  9. Psychosexual Service

    We specialise in treating people with chronic and persistent sexual problems. Our service offers assessment and treatment for individuals and couples who are experiencing difficulties with their sexual functioning, where the primary cause is psychological, or where the psychosexual problem has developed as a result of physical pathology.

    We have a longstanding history of treating people psychologically for persistent difficulties relating to sex and gender. We can treat people with significant distress including those suffering from co-morbid depression or anxiety.

    Our aim is to assist people to improve their sexual wellbeing and overall quality of life. We have expertise in treating people with a range of sexual dysfunctions which includes:

    • anorgasmia
    • delayed or absent ejaculation
    • erectile dysfunction
    • genital pain disorders, such as dyspareunia, vaginismus and vulvodynia
    • persistent genital arousal disorder
    • premature ejaculation
    • sexual aversion
    • sexual desire disorders; low or excessive sexual drive
    • Other sexual difficulties:
      • fetishism and other unusual sexual practices
      • female genital mutilation (FGM)
      • pornography or cybersex addiction
      • sexual addiction
      • sexual trauma
    • Psychological support for people experiencing gender dysphoria

    Our Psychosis Community Services provides care for adults, aged 18-65, who have a psychotic illness. This involves distorted perceptions of reality - thinking, feeling, hearing and seeing - often with symptoms of hallucinations and delusions.

    We are available to people who live in the London Borough of Lewisham and already receive treatment for mental illness from one of our other therapy services.

    People eligible for our service must be on the Care Programme Approach (CPA). The CPA is for patients who need to see several people or organisations for their mental health care and treatment.

    We help people to continue with the improvements they have made with their illness, teaching them the skills to live independently in their community.

    We work with a team of health and social care professionals including nurses, doctors, social workers, occupational therapist psychologists. We also work closely with the patients, their carers and GPs, Benefit workers , Social inclusion recovery service (SIRS), Lewisham AMHP service, local day care centres and housing associations and supported living placements as required.
